ABOUT US

We are a Semi Independent home proving accommodation and support from young people aged 16 - 23 years. Our service offer tailored support from high-level support packages offering 24-hour support – to low-level support. We are also an outreach service providing support to young people in their own accommodation.

Our service offers placements to young people leaving care and our key focus is to ensure that all our young people are empowered to achieve realistic goals and ambitions in relation to their transition into independent living. 

What We Offer:

  • Transitional support packages: Offering continuity of care for the full care journey of the young person.  Providing a smooth, child-centered, and coordinated transition from semi-independent to Independent services.

  • Low-level support package: Accommodation with a minimum of 15 hours per week within a solo provision. 

  • Medium-level support packages: Accommodation and overnight staff presence in addition to daily support sessions. 

  • High-level support packages: Accommodation and 24/7 provision and implementation of a phased reduction plan. 

  • Intensive support packages: Accommodation with high-level support, including overnight sleep or waking night support hours to minimise risks. With an additional support plan aimed at a 6-month reduction model. 

  • Emergency placements: supported provisions available to transition in an emergency. 

  • Group Living: Accommodation offering a private room with shared areas such as the kitchen with on-site staff to provide support with additional commissioned hours in line with young people’s support needs.

  • Parent and baby placements: These will be considered on an individual basis and will depend on the needs of the parent and/or baby

  • Family Placements: To support parents to live independently with outreach support to suit the needs of the parents. The service is unable to provide support to children and a robust family support plan is to be provided by social services.

  • Support Only: Mentoring and outreach support.
